RN Care Coordinator

The Community Care RN Care Coordinator is responsible for organizing and directing team members to achieve desired patient outcomes through day-to-day and/ or telephonic care coordination and management. The RN Care Coordinator uses the nursing process and evidence-based practice to collaborate with the Integrated Team and Provider teams (VA and Community) in developing the patient-driven care plan.
