Social Worker - Health Behavior Coordinator (HBC)

The HBC serves as the facility's subject matter expert and consultant on patient-centered communication and on supporting patients in improving self-management through optimized health behaviors. The HBC provides and supports clinical services targeting behaviors such as tobacco use, stress management, weight management, and insomnia. In addition, the HBC is responsible for staff education in patient-centered communication and related trainings that promote improved health outcomes.
