RN Case Manager (field position in Newark & surrounding areas)

Position Responsibilities:

• Will be assigned to 30 members per week that will need to follow up on and perform assessments/care plans

• Entirely field based and working from home- telephoning in one time per week for meetings

• Evaluates members for case management services and determines appropriate level of care coordination/ management services for member

• Develops and monitors members plan of care to include progress toward meeting established goals and self-management activities
